Supporting self care in primary care - the book
This book is a guide for anyone involved in commissioning or providing primary care, to help them understand what people can do to care for themselves or those they look after. Then they can set up systems in the primary care organisation (PCO) or their practice to support self care and enable patients and the public as a whole to self care whenever possible.
The book is a part of the Self Care for Primary Care initiative toolkit encouraging health professionals to learn to promote self care, and introduce self care support interventions
for common health conditions in their patients. Chapter 14 provides more information on how to use the tools featured in the workshops.
