Roles and responsibilities checklist
Fill in the tasks/roles that need to be completed to establish a culture that supports self care. Allocate a person to carry out each task/role, add in the date for completion and then tick once the task is completed.
This chart needs to be a ‘living document’ that is added to after each training session with tasks that are allocated to relevant people as the culture to support self care develops within your PCT/practice/care agency.
