Why you should use this tool
This tool will help you to assess the readiness of your organisation’s infrastructure to implement an action plan. The matrix should help you to plot gaps in resource availability.
When to use this tool
Whilst taking stock of your baseline resources in the preliminary stages of evolving an action plan for a new initiative such as supporting self care among patients.
What to doUsing the table overleaf, plot your PCT/practice/care agency resources by ticking where there are identified resources available and making a cross where there are none. |

Time:approximately 1 hour with additional time to talk to the relevant people in your PCT/practice/care agency to help you fill in any gaps. |

How it works (insight)
Identifying gaps in infrastructure or resources enables you to begin to build an action plan of the next steps in your journey towards supporting self care as an organisation.

Feed the completed table into the action planning process.
